A. Executing a Dematerialization Form without certificates

• Completed share Dematerialization Form duly executed by the investor

• Indemnity portion of the Dematerialization Form to be signed by the Managing Director of the presenting stockbroking firm.

• Passport photograph of the shareholder duly endorsed by the stockbroker

• Means of Identification (Government issued photo ID)

• CSCS Statement of Account

• Board Resolution and signature mandate where the investor is a coporate.

B. Executing a Dematerialization Form with Certificates

• Completed full share dematerialization form duly executed by the investor

• Passport photograph of the shareholder duly endorsed by the stockbroker

• Means of Identification (Government issued photo ID).

• CSCS Statement of Account (not less than 3 month)

• Indemnity portion of the Dematerialization Form to be signed by the Managing Director of the presenting stockbroking firm.

• Board Resolution and signature mandate where the investor is a coporate.
